A cardiac stress test is a medical test that indirectly reflects arterial blood flow to the heart during physical exercise. When compared to blood flow during rest, the test reflects imbalances of blood flow to the heart&#8217;s left ventricular muscle tissue – the part of the heart that performs the greatest amount of work pumping blood.<br />
The results may also be interpreted as a reflection on a person&#8217;s overall physical fitness.<br />
The first standardized cardiac stress test was developed in 1929 by Arthur Master, a doctor at Mount Sinai Hospital in New York City.<br />

How is a stress test performed?<br />
First, you will have leads (wires) to an ECG machine attached to your chest, and a blood pressure cuff is placed on your arm. A clothespin-like sensor may be placed on your finger to measure the amount of oxygen in your blood. After a baseline ECG is obtained, you will be asked to begin performing a low level of exercise, either by walking on a treadmill, or pedaling a stationary bicycle. The exercise is &#8220;graded&#8221; &#8211; that is, every three minutes, the level of exercise is increased. At each &#8220;stage&#8221; of exercise, your pulse, blood pressure and ECG are recorded, along with any symptoms you may be experiencing.<br />
With a &#8220;maximal&#8221; stress test, the level of exercise is gradually increased until you cannot keep up any longer because of fatigue, or until your symptoms (chest pain, shortness of breath, or lightheadedness) prevent further exercise, or until changes on your ECG indicate a cardiac problem. Maximal stress tests should be performed when the goal is to diagnose the presence or absence of CAD.</p>
<p>With a &#8220;submaximal&#8221; stress test, you will exercise only until a pre-determined level of exercise is attained. Submaximal tests are used in patients with known CAD, in order to measure whether a specific level of exercise can be performed safely.</p>
<p>After the test, you will be monitored until any symptoms disappear, and until your pulse, blood pressure and ECG return to baseline.</p>

What is Chondromalacia patellae?<br />
Chondromalacia literally means &#8220;softening of the cartilage&#8221;, and Patellae means &#8220;the knee-cap&#8221;. So Chondromalacia patellae means &#8220;softening of the articular cartilage of the knee-cap.&#8221; The articular cartilage is the cartilage lining under the knee-cap that articulates with the knee joint. Under normal circumstances, it is smooth and shiny, so that it glides smoothly along the articular groove of the femur as the knee bends. When it &#8220;softens&#8221;, it may break down, causing irregularities along the undersurface of the patella.<br />
<span id="more-440"></span><br />
The ends of bones which form joints are covered by a very smooth material called articular or chondral cartilage. After injury or with disease, the condition of this surface can change.  This can progress over time from becoming a bit soft to being completely absent.  The loss of articular or chondral cartilage of a joint for any reason, (injury, illness, or natural wear- and- tear with aging), is called chondromalacia.</p>
<p>These changes compare to the difference seen between a brand-new Teflon-coated frying pan, one that has been slightly scratched by a steel scrubber, and one where the Teflon surface has flaked off completely.  As with a frying pan&#8217;s Teflon surface, the greater the destruction of the articular cartilage, the poorer the function of the joint.</p>
<p>Important considerations when evaluating chondromalacic changes in a joint are the state of the articular surface, the extent of the cartilage involvement and the location of the lesion.  These all impact the degree of symptoms.</p>
<p>There are several classification systems used to grade the degree of articular cartilage damage. These usually describe the extent of fibrillation of articular surfaces, the degree of softness, the depth of involvement and sometimes the size of the lesion.  Some of the systems combine surface appearance and the depth of involvement under a single category and others make no such distinction. There is some subjectivity in reporting but all classification systems attempt to be as objective as possible.</p>
<p>Generally, <strong>a Grade I</strong> lesion is a descriptive term for articular cartilage which is soft.  <strong>Grade II</strong> lesions usually show some fissuring or disruption of the articular surface to various depths.  <strong>Grade III </strong>lesions are more extensive with deeper fissuring and fibrillation which may extend to the underlying subchondral bone.  <strong>Grade IV </strong>is a descriptive term used to describe that the articular cartilage is destroyed, leaving the subchondral bone exposed (eburnation).  Grade IV is the most objective of the grades, and is the most consistent across the classification systems.</p>

Since in humans the knee supports nearly the whole weight of the body, it is the joint most vulnerable to both acute injury and the development of osteoarthritis.<br />

It is often grouped into tibiofemoral and patellofemoral components.[4][5] (The fibular collateral ligament is often considered with tibiofemoral components.)[6]</p>
<p>The Knee:<br />
The knee joint, medically known as the tibiofemoral joint, is the largest joint in the body. Two bones make up this joint; the femur and tibia. This joint is dependent on the muscles and ligaments which surround it for strength.</p>
<p>The Ligaments:<br />
Four ligaments are present in the knee joint, the medical collateral ligament, lateral collateral ligament, anterior cruciate ligament, and posterior cruciate ligament. These ligaments provide strength to the knee joint.<br />
The medial collateral ligament is located at the inside of the knee joint. It extends from the medial femoral epicondyle to the tibia. This ligament prevents excessive abduction of the knee.</p>
<p>The lateral collateral ligament is located at the outside of the knee joint. It extends from the lateral femoral epicondyle to the head of the fibula. This ligament prevents excessive adduction of the knee.<br />
The anterior cruciate ligament extends posterolaterally from the tibia and inserts on the lateral femoral condyle. This ligament prevents excessive posterior movement of the femur on the tibia.</p>
<p>The posterior cruciate ligament extends anteromedially from the tibia posterior to the medial femoral condyle. This ligament prevents excessive anterior movement of the femur on the tibia.<br />

Functional Knee Braces<br />
Functional knee braces are designed to substitute for damaged ligaments. For example, a patient who sustains an ACL tear may be offered a knee brace to wear in efforts to allow certain activities without surgery.</p>
<p>Prophylactic Knee Braces<br />
Prophylactic knee braces are used to prevent knee injuries. Prophylactic knee braces are worn by athletes who participate in some high-risk sports in an effort to minimize their risk of sustaining a knee injury.<br />
Functional Knee Braces<br />
Most patients who are concerned about knee braces already have a knee ligament injury. These patients would be interested in the functional knee braces. These functional knee braces are designed to compensate for a torn knee ligament.<br />
Do the functional knee braces work as well as a normal ligament?<br />
This is an easy question to answer: No. However, the functional knee braces may help in patients who have a knee ligament injury. There have been a number of studies to investigate the effectiveness of these knee braces. The summary of these studies is that functional knee braces provide some protection to the knee at low loads. This means that when a force is applied to a knee that is supported with a functional knee brace, it is more stable than without the brace.</p>
<p>So doesn&#8217;t that mean that functional knee braces work?<br />
Not quite. The forces that are applied in these studies are thought to be much lower and more slowly applied than forces applied to the knee during competitive athletics. When a knee injury occurs during high-level sports, the knee is subjected to very high forces that occur very quickly. Unfortunately, testing these braces under these unique conditions is difficult. No one can prove that functional knee braces are of much help under these unique conditions.</p>
<p>Should I wear a functional knee brace if I have a ligament injury?<br />
You should discuss this with your doctor, because it depends on several factors, including:</p>
<p>Which ligaments are injured<br />
What sport you are participating in<br />
What rehabilitation you have done or plan to do<br />
What is known, it that physical therapy, including strengthening, is an important factor in returning to sports following a ligament injury. Therapy and rehabilitation are by far more important than a knee brace. Knee braces may help prevent re-injury to the knee, but it is likely a small factor, if any at all.<br />
Prophylactic Knee Braces<br />
Prophylactic knee braces are designed to prevent knee injuries in healthy athletes. These knee braces were popularized in the late 1970s when the use of prophylactic knee braces was tested in NFL players. Since that time, several studies have investigated the injury rates in athletes who wear prophylactic knee braces versus those who don&#8217;t wear a knee brace.<br />
Do athletes who wear knee braces have fewer injuries?<br />
Again, the evidence is cloudy, because the difference is very small. Studies do show that athletes in certain sports (football) have a lower rate of MCL injury when wearing a prophylactic knee brace. However, the effect of the prophylactic knee brace should be placed in perspective. Much more important factors in determining the likelihood of an injury include:</p>
<p>Sport played<br />
Player position<br />
Player conditioning<br />
Player size<br />
Is there any harm in wearing a prophylactic knee brace?<br />
There was some concern that knee braces could alter the forces on the knee such that prophylactic knee braces could prove problematic. However, knee braces, when properly fitted and worn, have not been shown to increase injury rates. They are probably safe to wear, and have not been shown to increase rates of knee or ankle injuries.<br />
Where should I obtain a knee brace?<br />
The knee braces that are investigated in these studies are not simple knee sleeves or knee braces that can be purchased at a drug store. These knee braces need to be special ordered and prescribed by your physician. If you are interested in using a knee brace for competitive sports, you should contact your team doctor, or your orthopedic surgeon. Your doctor can prescribe the proper type of knee brace for you and your sport. Furthermore, any effective knee brace must fit properly. Once you have a knee brace, ensure that your doctor inspects the fit of the knee brace to ensure it is sized and placed properly.</p>

With gas prices trending downwards, the annual family road-trip vacation could be making a comeback this year. But before you hit the highways, you will want to think about a few things to make your journey safer and more enjoyable.<br />

Be Safe on the Road</p>
<p>Planning a road trip this holiday season or beyond? From where you&#8217;ll stay each night to which sights you&#8217;ll visit along the way, you&#8217;ll have lots of decisions to make. If you&#8217;ll be traveling with your spouse, other family members, or friends, you can count on making lots of compromises too. After all, everyone has a slightly different idea of what makes for the perfect  vacation.</p>
<p>But there&#8217;s one thing you and all your travel companions can agree on: Getting to your destination and back safely is priority number one. So take a few minutes now to focus on what matters most. By simply following the 3 steps outlined below, you can map out a strategy to avoid road trouble and ensure your entire trip is as safe and worry-free as possible.</p>

<p>1. BEFORE YOU LEAVE</p>
<p>Regular maintenance such as tune-ups, oil changes, battery checks, tire rotations, and so forth go a long way toward preventing breakdowns before they happen. If your vehicle has been serviced according to the manufacturer&#8217;s recommendations, it should be in good shape and nearly ready to travel. If not &#8212; or you don&#8217;t know the service history of the vehicle you plan to drive &#8212; schedule a preventive maintenance check-up with your mechanic now.</p>
<p>Providing your vehicle is well maintained, getting it ready for a road trip is relatively quick and easy. However, it&#8217;s important to perform the following basic safety checks before you go:</p>
<p>Vehicle Safety Checklist</p>
<p>Tires &#8212; Air pressure, tread wear, spare</p>
<p>Check the air pressure in all your tires, especially the neglected spare. To get an accurate reading, check pressure when tires are cold, meaning they haven&#8217;t been driven on for at least three hours. It&#8217;s a good idea to keep a tire pressure gauge on hand in your vehicle for this purpose. You can find the correct pressure for your tires listed on a label inside the driver&#8217;s doorframe or in the vehicle&#8217;s owner&#8217;s manual &#8212; the correct pressure is not the number listed on the tire itself.</p>
<p>Also, take five minutes to inspect your tires for signs of excessive or irregular wear. If the tread is worn down to 1/16 of an inch, it&#8217;s time to replace your tires. If you find irregular tread wear patterns, it means your tires need rotation and/or your wheels need to be realigned before you leave. For more information on tire safety and pressure, visit the &#8220;Tires&#8221; section of www.safercar.gov, a vehicle safety resource of the National Highway Traffic Safety Administration (NHTSA).</p>
<p>Belts and Hoses &#8212; Condition and fittings</p>
<p>Look under the hood and inspect all belts and hoses to make sure they are in good shape with no signs of blisters, cracks, or cuts in the rubber. High summer temperatures accelerate the rate at which rubber belts and hoses degrade, so it&#8217;s best to replace them now if they show signs of obvious wear. While you&#8217;re at it, check all hose connections to make sure they&#8217;re secure.</p>
<p>Wiper Blades &#8212; Wear and tear on both sides</p>
<p>After the heavy duty imposed by winter storms and spring rains, it&#8217;s likely that your windshield wipers are ragged from use and ready to be replaced. Moreover, like rubber belts and hoses, wiper blades are susceptible to the summer heat. Examine your blades for signs of wear and tear. If they aren&#8217;t in tip-top condition, invest in new ones before you go.</p>
<p>Cooling System &#8212; Needed servicing and coolant level</p>
<p>Carefully check your coolant level to make sure it&#8217;s adequate. In addition, if it&#8217;s time to have your cooling system flushed and refilled (or even nearly time), have it done now. On a long road trip in summer heat, you&#8217;ll want your cooling system functioning at peak performance to avoid the possibility of your engine overheating.</p>
<p>Fluid Levels &#8212; Oil, brake, transmission, power steering, coolant, and windshield</p>
<p>Obviously, you&#8217;ll want to check your vehicle&#8217;s oil level. And as with coolant, if it&#8217;s time or even nearly time to have the oil changed, now would be a good time to do it. In addition, check the following fluid levels: brake, automatic transmission, power steering, windshield washer, and coolant. Make sure each reservoir is full and if you see any sign of fluid leakage, take your vehicle in to be serviced.</p>
<p>Lights &#8212; Headlights, brake lights, turn signals, emergency flashers, interior lights, and trailer lights</p>
<p>See and be seen! Ensure all the lights on your vehicle are in working order. Check your headlights, brake lights, turn signals, emergency flashers and interior lights. Towing a trailer? Be sure to check your trailer lights too. Failure of trailer light connections is a common problem and a serious safety hazard.</p>
<p>Air Conditioning &#8212; Sensitive to heat? Check AC performance before traveling</p>
<p>If you&#8217;re traveling with someone sensitive to heat, you may also want to ensure that your air conditioning system is functioning properly. Lack of air conditioning on a hot summer day can adversely affect people who are in poor health or are sensitive to heat, such as children and seniors. If the air is not blowing cold, it&#8217;s a good idea to have the system repaired before you go, especially since emergency, on-the-road repairs can be more costly than those you plan in advance.</p>
<p>2. WHAT TO TAKE</p>
<p>Child Safety Seats </p>
<p>If you&#8217;ll be traveling with children, you&#8217;ll need to make room in your back seat for appropriate child safety and/or booster seats. NHTSA guidelines for proper child safety and booster seat use can be found at www.boosterseat.gov under &#8220;Which seat is right for my child?&#8221;</p>
<p>Emergency Roadside Kit </p>
<p>Even a well-maintained vehicle can break down, so it&#8217;s advisable to put together an Emergency Roadside Kit to carry with you. A cell phone tops the list of suggested emergency kit contents since it allows you to call for help when and where you need it. Suggested Emergency Roadside Kit contents include:</p>
<p>• Cell phone</p>
<p>• First aid kit</p>
<p>• Flashlight</p>
<p>• Flares and a white flag</p>
<p>• Jumper cables</p>
<p>• Jack (and ground mat) for changing a tire</p>

<p>• Basic repair tools and some duct tape (for temporarily repairing a hose leak!)</p>
<p>• A jug of water and paper towels for cleaning up</p>
<p>• Nonperishable food, drinking water, and medicines</p>

<p>3. ON THE ROAD</p>
<p>Getting there is half the fun of a road trip vacation. It&#8217;s also half the hassle. Plenty of distractions both inside and outside your vehicle can take your attention from the road. Cranky fellow travelers, traffic congestion, road construction and detours, ringing cell phones, unfamiliar roads, and loud music are just a few you might encounter. While you can expect some distractions, you can also counter them with safe driving techniques to help you keep your focus.</p>
<p>Tips for Staying Focused </p>
<p>The best way to stay focused while driving is to avoid fatigue. So schedule your trip to allow for frequent breaks. Stopping for food or beverages, taking time to pull over at a rest stop just to stretch your legs, staying overnight at a motel or local bed and breakfast, and sharing the driving are all good strategies for avoiding fatigue and staying alert behind the wheel.</p>
<p>Traveling with Children </p>
<p>Remember that long trips can be especially tough on children. Stopping along the way makes the trip easier and less tiring for them as well &#8212; and more of an adventure too! Also, bring along a few favorite books or soft toys to keep the little ones content and occupied. <p>Keep in mind that the best wat to prevent skin cancer is to protect yourself from the sun&#8217;s UV rays. This means staying out of the sun at peak times, wearing proper sunscreen and protective clothing when outdoors.</p>
<p>If you think you may have skin cancer, please get checked out by a healthcare professional. The earlier skin cancer is detected, the better the survival rate.</p>
