			<content:encoded><![CDATA[<p>People who have a <strong>swollen knee</strong> can either have fluid within the knee joint itself, or in the surrounding soft tissue. The first step in determining the cause of the fluid accumulation is to determine where the fluid is located.  <span id="more-866"></span></p>
<p>The knee joint is surrounded by a capsule. This capsule forms the &#8220;joint space.&#8221; The joint space normally has a small amount of lubricating fluid within the knee to help keep the knee moving easily. Some conditions cause an accumulation of this fluid. This is what most people consider a swollen knee, also called a knee effusion. </p>
<p>There are a number of <strong>causes of knee swelling</strong>, the most common of which is arthritis. Other causes include the following:</p>
<p>    * Infection (infection of the knee is serious and requires swift medical treatment)<br />
    * Pregnancy<br />
    * Blood clots<br />
    * Trauma or injury to the knee (e.g., after a fall or a cruciate ligament, meniscal, or collateral ligament tear )<br />
    * Dehydration<br />
    * Obesity or being overweight<br />
    * Water retention or fluid buildup from infection, blood or joint fluid<br />
    * Poor circulation<br />
    * Insect bite<br />
    * Menopause<br />
    * Menstrual cycle<br />
    * Multiple sclerosis<br />
    * Mono<br />
    * Autoimmune disease (e.g. rheumatoid arthritis)<br />
    * Hypothyroidism<br />
    * Bursitis<br />
    * Gout<br />
    * Tendonitis<br />
    * Chondromalacia patella (a condition that causes a softening of the cartilage)<br />
    * Over-use or strain during exercise</p>
<p><strong>Treatment </strong><br />
• Exercises are important.  Many knee conditions will improve with conservative treatment consisting of low impact activities and exercises to improve muscular strength and flexibility. Patellofemoral dysfunction is best treated with vastus medialis strengthening and hamstring and calf stretching [B*] .<br />
•  In most cases a home treatment program should be explained in detail to the patient, including specific guidelines for activity modification and exercises.  Initially, formal physical therapy is usually not required.<br />
•  All patients with mild to moderate knee osteoarthritis who do not have medical contraindications should be offered an exercise program that includes lower extremity strengthening and stretching exercises combined with low impact aerobic exercises (e.g., swimming, biking, walking, cross- country skiing) [A*] .<br />
•  The initial drugs of choice for the treatment of the pain of knee osteoarthritis are acetaminophen and/or topical capsaicin [A*] .  If a traditional NSAID is indicated, the choice should be based on cost (see Table 6).  COX-2-Inhibitors are no more effective than traditional NSAID agents; they may offer a short-term but probably no long-term advantage in GI tolerance for some patients. Due to cost and potential cardiovascular effects, COX-2 inhibitors should be reserved for carefully selected patients .</p>

I hurt my right knee while running. Anyways, I am biking now and I already know it is supposed to be easier on your knees. cycling will strengthen your legs in general. actually, your knees have no muscles to strengthen. what you do is build the attaching muscles and it tightens the tendons and parts of the knee.<br />
cycling can destroy knees if done wrong. you must have your saddle up, so your knees are almost straight at the bottom of the pedal stroke. you should spin lightly, and avoid pushing hard on the pedals, and you should make sure your legs are correctly aligned. a bike shop can help with all these things. cycling works your knees a lot, and if something isnt right all that work can quickly damage them. if done correctly, cycling will build your legs very well.</p>

Thomas and colleagues allocated patients with self reported knee pain to receive one of four options: a home based exercise programme, telephone support, exercise and telephone support, or standard care. Participants who exercised reported a 12% reduction in knee pain compared with those who did not exercise.<br />

<strong>Effectiveness of home exercise on pain and disability from osteoarthritis of the knee: a randomised controlled trial</strong><br />
OBJECTIVE To assess the effect of a home based exercise programme, designed to improve quadriceps strength, on knee pain and disability.</p>
<p>Methods—191 men and women with knee pain aged 40–80 were recruited from the community and randomised to exercise (n=113) or no intervention (n=78). The exercise group performed strengthening exercises daily for six months. The primary outcome measure was change in knee pain (Western Ontario McMaster Osteoarthritis index (WOMAC)). Secondary measures included visual analogue scales (VAS) for pain on stairs and walking and WOMAC physical function scores.</p>
<p>Results—WOMAC pain score reduced by 22.5% in the exercise group and by 6.2% in the control group (between group difference p<0.05, unpairedt test).VAS scores for pain also reduced in the exercise group compared with the control group (p<0.05). Physical function scores reduced by 17.4% in the exercise group and were unchanged in controls (p<0.05).</p>
<p>Conclusion—A simple programme of home quadriceps exercises can significantly improve self reported knee pain and function. </p>

<p>Participants: 786 men and women aged—45 years with self reported knee pain.</p>
<p>Interventions: Participants were randomised to four groups to receive exercise therapy, monthly telephone contact, exercise therapy plus telephone contact, or no intervention. Patients in the no intervention and combined exercise and telephone groups were randomised to receive or not receive a placebo health food tablet.</p>
<p>Main outcome measures: Primary outcome was self reported score for knee pain on the Western Ontario and McMaster universities (WOMAC) osteoarthritis index at two years. Secondary outcomes included knee specific physical function and stiffness (scored on WOMAC index), general physical function (scored on SF-36 questionnaire), psychological outlook (scored on hospital anxiety and depression scale), and isometric muscle strength.</p>
<p>Results: 600 (76.3%) participants completed the study. At 24 months, highly significant reductions in knee pain were apparent for the pooled exercise groups compared with the non-exercise groups (mean difference -0.82, 95% confidence interval -1.3 to -0.3). Similar improvements were observed at 6, 12, and 18 months. Regular telephone contact alone did not reduce pain. The reduction in pain was greater the closer patients adhered to the exercise plan.</p>
<p>Conclusions: A simple home based exercise programme can significantly reduce knee pain. The lack of improvement in patients who received only telephone contact suggests that improvements are not just due to psychosocial effects because of contact with the therapist. </p>

Unlike the other joints which are cushioned nicely with a lot of flesh like the hip joint, knee joint is a lot more exposed and thus takes the brunt of any falls or mishaps that we might have.<br />
Knees are weight bearing joints which help us in everything right from standing up to running and walking. They twist, turn and roll in order to accommodate our movements.<br />
This makes this joint very susceptible to being over uses and thereby injured.<br />
Knee pain is very common and can be caused by an injury to any of the ligaments or cartilages surrounding the knee. This is by no means the only reason you could end up with acute pain in the knee joints.</p>
<p>The Ways to Handle Knee Pain:<br />
<strong>Rest</strong><br />
If knee pain is a persistent problem, it&#8217;s important to reduce the stress to it whenver possible. To limit the pain felt when walking, use orthotic shoe insoles in every pair of shoes. This often results in instant pain relief when walking by redistributing the placement of your weight on the knee.<br />
<strong>Swelling Management</strong><br />
Whether due to injury or arthritis, knee pain is often accompanied by infrequent swelling. Even if the knee wasn&#8217;t sore before, the swelling may cause an uncomfortable flare-up. When you notice the knee beginning to swell, elevate the foot and knee off the ground and use an ice pack on the knee. PhD Mark J. Yaffe, professor at McGill University in Montreal, reports that utilizing ice for pain increases blood flow to the affected area and results in faster healing.<br />
<strong>Exercise</strong><br />
A strong leg limits the amount of stress placed on the knee. For this reason, many physical therapists recommend treating knee pain with leg exercises. Do low-impact exercises that work the thigh muscles to improve long-term knee pain problems. If every exercise hurts your knee, try clenching and releasing the thigh muscles while you rest. This will slowly improve quadriceps strength.<br />
<strong>Weight Loss</strong><br />
One of the largest contributing factors of knee problems is being overweight. Even a 10-lb. weight loss will go a long way toward improving knee pain.<br />
<strong>Bracing</strong><br />
Taping and bracing is often suggested to properly support the knee. Elastic wraps and braces offer additional benefits, as they compress the injured knee as well as support it. However, even a little medical tape along the side of the affected knee will help keep the knee from moving improperly. </p>

Now that you know all about its working parts, you probably realize that there are a bunch of ways to injure a knee. Common causes for injuries are overuse (from repetitive motions, like in many sports), sudden stops or twists, or direct blows to the knee.</p>
<p>Here are some of the more common injuries:<br />
<span id="more-337"></span><br />
Sprains<br />
A sprain means you&#8217;ve stretched or torn a ligament. Common knee sprains usually involve damage to the ACL and/or MCL. The most serious sprains involve complete tears of one or more of the knee ligaments. Symptoms of knee sprains include:</p>
<p>a popping or snapping sound in the knee at the time of injury<br />
pain that seems to come from within the knee, especially with movement<br />
not being able to put any weight on that leg<br />
swelling<br />
fluid behind the kneecap<br />
the knee feels loose or unstable or gives way<br />
Strains<br />
A strain means you&#8217;ve partially or completely torn a muscle or tendon. With knee strains, you may feel symptoms similar to a sprain and may see bruises around the injured area.</p>
<p>Tendinitis<br />
Tendinitis happens when a tendon gets irritated or inflamed. It is often caused by overuse. A person with tendinitis might have pain or tenderness when walking, or when bending, extending, or lifting a leg.</p>
<p>Meniscal Tears<br />
Damage to the menisci is a really common sports injury, especially in sports where sudden changes in speed or side-to-side movements can cause them to tear. Meniscal injuries often occur together with severe sprains, especially those involving the ACL.</p>
<p>Meniscal injuries can cause tenderness, tightness, and swelling around the front of the knee. Sometimes fluid collects around the knee (this is called effusion).</p>
<p>Fractures and Dislocations<br />
A fracture is a cracked, broken, or shattered bone and is usually diagnosed by an X-ray. You may have trouble moving that bone and it&#8217;s likely there&#8217;s a lot of pain. Patellar dislocation happens when the patella (the kneecap) is knocked off to the side of the knee joint by twisting or some kind of impact. Sometimes it will go back to its normal position by itself, but usually it will need to be put back into place by a doctor.</p>
<p>Symptoms include swelling and a lot of pain at the front of your knee. There will usually be an abnormal bulge on the side of your knee, and you may be unable to walk.</p>
<p>Cartilage Injuries<br />
Sometimes a small piece of bone or cartilage softens or breaks off from the end of a bone, causing long-term knee pain. This is called osteochondritis dessicans (OCD). Symptoms of OCD include pain, swelling, an inability to extend the leg, and stiffness, catching, or popping sensations with knee movement. Treatment can include resting the knee, wearing a cast for a couple of months, and sometimes surgery in older teens.</p>
<p>Chondromalacia happens when the cartilage in the knee joint softens because of injury, muscle weakness, or overuse, and the patella and the thighbone may rub together. This causes pain and aching, especially when a person walks up stairs or hills. Treatment may involve surgery.</p>
<p>Symptoms<br />
The symptoms are different for the different forms of arthritis. For example, people with rheumatoid arthritis, gout, or other inflammatory conditions may find the knee swollen, red, and even hot to the touch. Any form of arthritis can cause the knee to be painful and stiff. </p>
<p>Pain and swelling of the knee results from knee injury. The knee joint may become unstable and make popping or snapping sounds that are generally accompanied by pain. </p>
<p>The knee joint can lock up. A traumatic event can produce intense knee pain. If the knee does not function properly, seek emergency medical help. </p>

The largest and most complicated joint, the knee is used for everything from standing up, sitting, to walking, running, etc. It&#8217;s a weight-bearing joint that straightens, bends, twists and rotates. All this motion increases your risk of acute or overuse knee injuries.<br />
<span id="more-335"></span><br />
Knee pain is incredibly common. In the United States, it&#8217;s responsible for about 1/3 of all doctor&#8217;s visits for muscle and bone pain. Knee pain is a special problem for athletes &#8212; over half of all athletes endure it every year.</p>
<p>Some of the most common reasons for knee pain are swollen or torn ligaments, meniscus (cartilage) tears, and runner&#8217;s knee. But the knee is a complex joint, and there&#8217;s plenty more that can go wrong.</p>
<p>Other conditions that <strong>cause knee pain</strong> are:</p>
<p>Bone chips. Sometimes, a knee injury can break off fragments from the bone or cartilage. These pieces can get stuck in the joint, causing it to freeze up. You may also have pain and swelling.<br />
Bursitis. A bursa is a sac of fluid that cushions and protects your joints. There are several in different parts of your knee. Overuse, a fall, or repeated bending can irritate the bursa, causing pain and swelling. Two types of bursitis are called &#8221;housemaid&#8217;s knee&#8221; and &#8221;preacher&#8217;s knee,&#8221; since they are often caused by kneeling. A &#8221;Baker&#8217;s cyst&#8221; &#8212; a swelling of one of the bursa in the knee &#8212; can also result from injuries and from conditions like arthritis.<br />
Iliotibial band syndrome. The iliotibial band is a piece of tough tissue that runs from your hip down to your shin. If it&#8217;s irritated by overuse or other problems, it can get swollen and cause pain on the outside of the knee.<br />
Medial plica syndrome. The plica is a fold of tissue in the knee joint. When it gets irritated from overuse, swelling and knee pain can result.<br />
Osgood- Schlatter Disease. This condition causes a painful bump below the knee, where a tendon from the kneecap connects to the shin. It&#8217;s usually caused by overuse and irritation of the tendon. The pain may come and go over time. It&#8217;s especially common in teenage boys and girls (13 and 14 years of age).<br />
Osteoarthritis. This condition is a frequent cause of knee pain in athletes and non-athletes alike who are over 60.<br />
Partially dislocated kneecap (or patellar subluxation). In this condition, the kneecap slides out of position, causing knee pain and swelling. It&#8217;s often the result of physical defect in your legs, rather than an injury. It&#8217;s particularly common in teenage girls.<br />
Tendonitis, or swelling of the tendons. Tendons are tough bands of tissue that connect your bones and muscles. Overuse can make the tendons inflamed and sore. One type of knee tendonitis is called &#8221;jumper&#8217;s knee.&#8221;<br />
If an old knee injury was not properly treated, it may keep causing occasional &#8212; or constant &#8212; knee pain.</p>

There are numerous causes of knee pain, for a  runner, it&#8217;s commonly caused by either a slightly misaligned hip bone that is throwing your gait off a bit resulting in extra stress on the knee or a weak hip/thigh muscle that is not properly stabilizing the hip during running that results in extra stress to the knee. In both cases, you may consult with a sports physician to locate what is causing the problem and the doc can refer you to a physical therapist so you can isolate and work on the offending area.<span id="more-302"></span><br />
When you feel your knee pain, have it checked out by an Orthopaedist or Physical Threapist. The condition that causes the pain is still there. It could be anything &#8211; Patello-femoral syndrome, osteoarthitis, et. al). Only your doctor can know for sure then you will know what to do for it.</p>
